We have an exciting opportunity in the University Health Service for a Data Care Coordinator to join our team. We are looking for an individual who has great communication skills and is keen to work as part of a team to provide excellent service to all of our colleagues and patients. You will be enthusiastic about looking after the needs of our student population.
You will work within our Data Quality Team to review and code patient documents and run clinical searches, in addition to a variety of administrative tasks. You will encourage patients to access support services required to keep them well.
You will be responsible for case finding patients eligible for a range of NHS screening and will proactively work with patients to encourage them to attend appointments by sharing health promotion information with them to allow them to make informed decisions regarding their care. You will also work closely clinical leads to help monitor and achieve targets set within a range of NHS contracts, including the Primary Care Network DES and our GP Practice Contract.

The University Health Service is an NHS GP practice which supports the health needs of the students of The University of Sheffield and their dependents. The team at UHS consists of GPs, Nurse Practitioners, Practice Nurses and Healthcare Assistants, as well as pharmacists, physiotherapists and other healthcare workers.
